お世話になります。

確かに
cgi.synck.com/mailform/pro2.1.1/example.html
こちらではエラーコンソールにエラーは出ません。素敵です。
(送信時にメールアドレスのエラーが出ますがこれはサンプルだからですよね？)

自サイトに設置した
figaro-web.com/sitetest/form/example.html
はいじらずにUPしております。ご指摘頂いたchecked="checked"のみ変更しております。
他javascriptも中身をいじっておりません(メールフォームプロを入れているフォルダだけformと名前を変えておりますが)が、example.htmlを開く際に三つエラーが出ます。

当方dreamweaverで製作しておりますが、example.htmlを開く際、include.cgiの1行目にシンタックスエラーがありますとのメッセージが出ます。
これは当方のパーミッション設定にでもミスがあると考えればよろしいでしょうか？

冗長で申し訳ありません、お手すきの際にでもご助言お願いいたします。
